Community Re-Entry Group

Originating in Outpatient Rehabilitation, this group travels with staff members to various locations in the community (transportation provided).

This group setting enables individuals to return to the community following a hospitalization and/or injury. After planning individual goals, the group travels to local establishments to address community mobility, functional cognition, money management and social interaction/awareness as well as safety/judgment. The group is led by an occupational therapist and a rehabilitation technician.

Typically covered by insurance when accompanied by a physician's prescription.
